Geriatric Service Integration Models HAMILTON NIAGARA HALDIMAND BRANT LHIN The Hamilton Niagara Haldimand Brant Local Health Integration Network brought together Planning Advisory Groups (PAGS) to advise the LHIN Clinical Services Plan Steering Committee on future models of service that will ensure the right care, in the right place, at the right time. The desired outcome is a system plan that will be operationalized over the next three to four years. The Division of Geriatric Medicine, McMaster University in collaboration with community stakeholders participated as one of 18 PAGS. Their clinical service plan was submitted to the LHIN in June, 2009 and recommendations from the LHIN Steering Committee are expected in October. At the request of the HNHB LHIN, GAIN engaged participants from various sectors adn all LHIN regions in focus groups during May-June to solicit information on service gaps, barriers to care etc. for patients needing SGS.
