Provides comprehensive assessment and consultation for the
older person with complex medical, psychiatric and/or functional
needs. Staff provide education to patients, caregiver systems
and formal health care providers.

focus on issues related to:
- Memory
- Continence
- Falls
- Osteoporosis
- Stay well (maintaining health)
- Mood disorders program

Provides assessment, initial treatment
and short-term rehabilitation
plan for the older person with complex clinical problems and
who requires an acute hospital admission with potential for
improvement. Includes patients with medical conditions with
associated psychiatric problems. |

older person with complex needs. The health care team works
with the patient, family, physician and other caregivers to
optimize the patients independence and quality of life. An
assessment may be provided in the community or in the outpatient
clinic. |
Provides inpatient consultation for elderly patients with
complex problems.
Assistance with discharge plans, management
of dementia and education
of staff, patients and their families. |
The purpose of the Geriatric Day Hospital is to help older
persons with complex health problems improve their health
and optimize functioning in the community. People who have
diseases typically related to aging, benefit from a multidisciplinary
assessment, including a medical review by a geriatrician,
and treatment for a variety of conditions.
Short term counseling
and education is also available to patients and their families. |
The Geriatric Assessment Unit (GAU) provides a complex
health assessment to identify conditions for which curative,
restorative, palliative or preventive treatment may be available.
The care plan includes an assessment of the patient’s
social support network, an assessment of the physical environment
and establishes appropriate goals and therapeutic plans. The
health care team makes recommendations to help the senior
function as independently as possible. |
