Enhancing Shared Care Approach for Frail Seniors:
New Resources for Primary Care


Provincially developed knowledge to practice, education and patient care resources and tools for the primary care of frail seniors are now available on more than 18 geriatric care, interprofessional and interorganizational topics.   

Frail seniors are unique because the complexities of their clinical presentations in primary care are such that optimal care requires knowledge in many key geriatric care issues such as dementia, falls, incontinence, delirium, polypharmacy;  interprofessional teamwork and interorganizational collaboration.
