About Best Practice Coordinators
(BP Coordinators in LTC)
The Regional BP Coordinator in LTC role was introduced into the long-term
care sector approximately two and a half years ago.It is funded by the
Nursing Secretariat with the goal of providing a regional best practice
resources for LTC homes and to introduce homes to evidence-based practice
and best practices.
There are eight BP Coordinators across Ontario helping LTC nurses and
staff use current health care knowledge and research to improve the
quality of the daily care provided to residents and families.
The Coordinators have developed a multitude of resources for LTC homes
seeking to implement best practices into their day-to-day care, including
everything from quick reference tools, assessments, care plans, policies,
procedures, education slides and resource kits for a number of care
issues (e.g., pain, falls, incontinence, resident-centred care, acute
change of condition).
Coordinators focus on sharing knowledge through on-site visits, consultations,
education, presentations, workshops, work groups and newsletters. They
will help locate the necessary resources homes need. Coordinators also
collaborate and synchronize with the many initiatives, networks and
organizations that interface with LTC.
